Christmas JOY Telethon Performer Information & Release Form

Living Ministries would like to thank you for supporting our community with the donation of your performance and expertise.

As part of this production we will provide:

Financial Compensation if dollars are available.
Your beverages purchased from The Electric Bean during Christmas JOY Telethon.
Promotion of your group on our website at ChristmasJOYTelethon.com and Facebook Pages for Living Ministries, Christmas JOY Telethon and committee members personal walls on Facebook, as well as Twitter.
An area for you to display and sell merchandise.
DVD recording of your performance at the time of the event. Please ask a technician in the sound booth during the show.
Exclaim Media Video & Marketing will edit and produce this show to be played on KLTV, from the footage shot by technicians at The Electric Bean. DVD copies of the production can be made available to you, at your request.
A Christmas JOY Telethon T-shirt can be provided for you at your request. Please send the sizes and preferred color needed for each interested person in your band so we can have them ready for you.

Living Ministries expects the following from each performer:

Musicians are free to bring their own food and non-alcoholic beverages to The Electric Bean. This is a drug and alcohol free zone. Smoking is only permitted 25 ft + away from the entrances of this establishment.
Please talk to Summer D Clemenson or Andrew Nguyen at The Electric Bean about band set up.
Musicians are expected to market this event to their groupies and followers. Please use the marketing materials at ChristmasJOYTelethon.com and make sure to “like” our event page at www.facebook.com/christmasjoytelethon so we can like your pages and promote you.
Each band is required to supply a table representative to handle sales and customer service for their merchandise. Living Ministries will not be responsible for your merchandise.
Please bring your own support team to help set up and tear down your equipment.
Set lists should be submitted before your performance. You may hand them to a Living Ministries representative before your performance. Any covers you perform will not be allowed in video productions played later on KLTV.

Video, Photo and Voice Release Form

For Valuable Consideration of my personal interview for the production of the video entitled: “Christmas JOY Telethon 2014” I, "Composer's Name (required)" , hereby irrevocably grant to Exclaim Media Video and Marketing their licenses, agents, successors and assigns, the right (but not obligation), in perpetuity throughout the world, in all media, now or hereafter known, to use (in any manner it deems appropriate, and without limitation) in and in connection to the Video, by whatever means exhibited, advertised or exploited: my appearance in the video, still photographs of me, my recordings of my voice taken or made of me by it, any music sung or played by me, and my actual or fictitious name.

On my own behalf, and on behalf of my heirs, next of kin, executors, administrators, successors and assigns, I hereby release Exclaim Media Video and Marketing, their agents, licensees, successors and assigns, from any and all claims, liabilities and damages arising out of the rights granted hereunder, or the exercise thereof.

NOTE: I understand that any video production produced by Exclaim Media Video and Marketing may be submitted for cablecast to KLTV or used on KLTV’s website.
